Error: Fcb::close-flush: Operating System Error 21(The Device Is Not Ready.) Encountered
May 23, 2007
We are using sql server 2005 Enterprise Edition with service pack1
I got the following error messages in the SQL log
The operating system returned error 21(The device is not ready.) to SQL Server during a read at offset 0x00000000090000 in file '....mdf'. Additional messages in the SQL Server error log and system event log may provide more detail. This is a severe system-level error condition that threatens database integrity and must be corrected immediately. Complete a full database consistency check (DBCC CHECKDB). This error can be caused by many factors; for more information, see SQL Server Books Online.
fcb::close-flush: Operating system error 21(The device is not ready.) encountered.
I got these errors for about 2 hrs and after that I see these messages in the sql log
Starting up database ' '
1 transactions rolled forward in database '' (). This is an informational message only. No user action is required.
0 transactions rolled back in database ' ' (). This is an informational message only. No user action is required.
Recovery is writing a checkpoint in database ' ' ( ). This is an informational message only. No user action is required.
CHECKDB for database '' finished without errors on (local time). This is an informational message only; no user action is required.
Can anyone please help me in troubleshooting this issue. Why this migh have happened.
I have recently installed a new server for a client running SBS2003 R2 with SQLServer 2005.
I noted that the backup failed the SQL Server database on the consistenty check. On checking the event log I see this message as per above.
It reads in the description
c:datalawsqldatadata41_Data.MDF:MSSQL_DBCC6:
Operating system error 112(error not found) encountered.
Looking in the error log I see :
Configuration option 'user options' changed from 0 to 0. Run the RECONFIGURE statement to install.
Error: 17053, Severity: 16, State: 1.
C:DataLAWsqldatadata41_Data.MDF:MSSQL_DBCC6: Operating system error 112(error not found) encountered.
DBCC CHECKDB (data41) WITH no_infomsgs executed by NT AUTHORITYSYSTEM found 0 errors and repaired 0 errors. Elapsed time: 0 hours 0 minutes 26 seconds.
This is a new server with about 90% of disk space NTFS still available
This is a problem I encountered when I had to detach a database file (type .mdf):
1) I went to the MS SQL Management Server Studi and detached my database file successfully from a connection called Workhorse.
2) I needed to place the .mdf database file into a zip file in order to put it on a remote server. I did this using Shared Portal. This was also successful
3) However when I tried reattaching the database file, I got this error:
CREATE FILE encountered operating system error 5A(Access denied.) while attempting to open or create the physical file "CProgram FilesMSSQL ServerMSSQLData<databasename>.mdf'
Q) The database file and log file (ldf) exist in the correct directory so I don't know what happened. Can any one help?
I have used the copy database wizard, but I realized I had forgotten to shrink the transaction log file. So I canceled the wizard. My database, detached by the wizard, has now disappeared. The mdf file is still there, but when I try to attach it manually I get the "create file encountered operating system error 5 while attempting to open the physical file..." error.
My apologies...I wasn't for sure where to post an error like this...
Over the last 2 months I have gotten this SQL Server error (twice). All existing processes will continue to work, however no new processes can be created and users cannot connect to the server. This is the exact text of the message in the SQL Server error log.

The SQL Server 2000 SP4 box with Windows 2003 Standard SP1 is well maintained by our I.T. team and it typically will run 4 or 5 months without a reboot.
Hi,I keep getting this error message for a trans.log backup.Operating system error112(error not found).The disk has about 6GB space free, and the backup should only take upabout 550 MB, so I would think it is not space related but...The disk is NTFS.Any ideas?
I have scripts to refresh database as SQL daily jobs. (O.S is Win2K3 and SQL server 2000 and SP4) It was worked and I got the following message this morning from SQL error log.
BackupMedium::ReportIoError: read failure on backup device '\XAPROD12MASTERXAPRODXAPROD_db_200701290000.BAK'. Operating system error 1450(Insufficient system resources exist to complete the requested service.).
Hi All,I use Bulk insert to put data to myTable.When the SQL server is in local machin, it works well. Hi, who have ever met the following errors, share an information of what could cause it??? I can't find any info that could help me find the reason. My DB went to SUSPECT mode and in the log file I got:
ForwardLogBlockReadAheadAsync: Operating system error 997(Overlapped I/O operation is in progress.) encountered.
Operating system error 997(Overlapped I/O operation is in progress.) on device 'd:MSSQLdataiservice_Log2.ldf' during ForwardLogBlockReadAheadAsync.
Error: 3414, Severity: 21, State: 1
Error: 9001, Severity: 21, State: 1
Unexpected error while commiting a ShrinkFile operation /*************************/
What is weird the 997 error occured twice during last night and the DB went into suspect mode only after the sencond one.
The database restored but it's a second time for the last month just getting a bit annoying.
Hi there. Could someone help me out. I'm performing a log restore and getting the following error within the SQL Server Error Log.
During restore restart, an I/O error occurred on checkpoint file 'G:MSSQL.1MSSQLBackupdxsh.CKP' (operating system error 1784(error not found)). The statement is proceeding but cannot be restarted. Ensure that a valid storage location exists for the checkpoint file.
IT's a very basic Restore command and i do have not specified it to Restart.
Having checked the MS website for 1784 it suggests the following:
The supplied user buffer is not valid for the requested operation
Can anyone tell me what is wrong here and why it is creating a checkpoint file or even what it is and how to overcome the error message.
i believe this is the right thread... i recently had an issue where my xpstar.dll would not register and i read to fix the issue to reapply the SP so i installed SP4 i now am getting this error... please help!
A customer of my program is getting the SQL CE error message "The operating system does not support encryption".
I've seen several threads on this issue, but none of them seem to apply to my problem. This is a Windows Forms application, so no device and no web service.
The OS is a German Windows XP - so there shouldn't be any issues regarding strong encryption. It even runs in Windows 2000 in our test VPC.
I am attempting to install SQL Server 2005 Developer Edition (from CD) on my IBM PC with XP Operating System. During set up, I get a error message (see below) telling me about a Service Pack requirement. When I click on the link provided in the error message, it does not lead to any service pack download.
I'm trying to read thru these forum posts, it seems that there is a service pack that I need to download after installing SQL Server 2005. But how can I install the pack after when I can't install SQL Server. Any help would be greatly appreciated. Below is more information about the version of windows my PC has.
My PC: Windows Version 5.1 (Build 2600.xpsp2.030422-1633:Service Pack 1)

I am trying to attach a database to SQL 2005. This database has not previously been attached. I have only just installed SQL.
I get the following message: Unable to open physical file "C:ArrowSQLArr@DataArrow_data.mdf" Operating system error 5: "5(error not found)" (Microsoft SQL Server: Error 5120)".
I have loaded SQL and the database fiel and directory with the same user account which is a local administrator on this machine. I have checked that I have read/write access to the file.
The machine runs Windows Vista Business. SQL has SP 2 loaded. What causes this?
I had few databases developed in SQL Server 2005 Developer Edition. I have detached all the databases and stopped developing anything new other than taking reqular backup of detached .mdf and .ldf files. I installed Vista on the other Hard drive that corrupted my XP installation drive letters so I had to reinstall XP OS. I have reinstalled SQL 2005 Dev edition. Now I am trying to attach the database it is giving error of Operating system error 5: "5(Access is denied.)". I tried to copy those .mdf and .Ldf files through windows explorer and it is not letting me copy past in the other place. I am getting Access denied from OS. I have about 5 databases and only one has this problem. What is going on and how do I solve this? Is the OS has locked the file? My home computer is not in a domain. I tried to right click the file to see if I can set access permission on the file. There is no such security options. Any help is appreciated.
We have a very severe issue in our production, and this is very very ugent to fix this issue, there was a job started on server on 10th October, 2007, for backup of database of 3.2 TB, which caused this issue on 18th October, 2007. I cancelled that job and even restarted the server, but still there is same issue on the server.
I am using this command to expand the physical file,

I am facing a issue with bulk upload on Test Server.
Issue: When running Openrowset command from SQL server other that Test Server query runs fine when trying to run the same command from Test Server it gives error.
Msg 4861, Level 16, State 1, Line 1
Cannot bulk load because the file "\ServerNameinputFileName.csv" could not be opened. Operating system error code 5(Access is denied.).
For example: If the command is run from System A connecting to SQL Server instance on Test Server Test Server it gives this error. If the same command with same rights is run from any other SQL server instance say Dev1 its running fine.
If the command is run from Test Server connecting to any SQL Server instance including Test Server it is running fine.
Tried: 1) Given the read/write rights on shared folder, to user under which the SQL server service is running on Test Server
2) Given the read/write rights on shared folder to everyone.
Query:
SELECT DISTINCT * FROM OPENROWSET
(
BULK '\ServerNameinputFileName.csv',
FORMATFILE='\ServerNameFormat.xml'
)
AS FileList
Please provide me with some solution. What can be the reason for such behaviour?
We are trying to attach a database in Sql Server 2005 Express on Windows Vista.It will Show the following Error.
"Unable to open the physical file "Test.mdf". Operating system error 5 :". And then we installed the SQL Server 2005 Service pack 2. This error again comes.Could you please give me the steps to resolve the problem.
Hello all, I am running into an interesting scenario on my desktop. I'm running developer edition on Windows XP Professional (9.00.3042.00 SP2 Developer Edition). OS is autopatched via corporate policy and I saw some patches go in last week. This machine is also a hand-me-down so I don't have a clean install of the databases on the machine but I am local admin.
So, starting last week after a forced remote reboot (also a policy) I noticed a few of the databases didn't start back up. I chalked it up to the hard shutdown and went along my merry way. Friday however I know I shut my machine down nicely and this morning when I booted up, I was in the same state I was last Wenesday. 7 of the 18 databases on my machine came up with
FCB:pen: Operating system error 32(The process cannot access the file because it is being used by another process.) occurred while creating or opening file 'C:Program FilesMicrosoft SQL ServerMSSQL.1MSSQLDataTest.mdf'. Diagnose and correct the operating system error, and retry the operation. and it also logs FCB:pen failed: Could not open file C:Program FilesMicrosoft SQL ServerMSSQL.1MSSQLDataTest.mdf for file number 1. OS error: 32(The process cannot access the file because it is being used by another process.).
I've caught references to the auto close feature being a possible culprit, no dice as the databases in question are set to False. Recovery mode varies on the databases from Simple to Full. If I cycle the SQL Server service, whatever transient issue it was having with those files is gone. As much as I'd love to disable the virus scanner, network security would not be amused. The data and log files appear to have the same permissions as unaffected database files. Nothing's set to read only or archive as I've caught on other forums as possible gremlins. I have sufficient disk space and the databases are set for unrestricted growth.
Any thoughts on what I could look at? If it was everything coming up in RECOVERY_PENDING it's make more sense to me than a hit or miss type of thing I'm experiencing now.
I have XP Pro SP2 with MDAC 2.8.1022. It had a problem so I tried to reinstall MDAC and got a Fatal Setup Error. This setup does not support installing on this operating system. I downloaded MDAC 2.8 1177 and get the same error.
I thought of uninstalling/reinstalling SP2, but this is a 2 month old Dell Latitude 610 with factory installed XP. There is no Windows Service Pack 2 option listed in the Control Panel > Add/Remove Programs.
There's some other strange things, so I wonder if they are related.
1) I have Paul set up as an administrator account. Some folders like MSSQL show that account with no permissions. I grant all the permissions to Paul for that folder. I come back later and the permissions are gone.
2) I deleted 20 files in Explorer, but 7 of them did not go away. I deleted those 7 again and they instatnly reappeared. I deleted those 7 again and then they finally went away.
3) I get a slow reaction time for things like Windows Explorer and opening and closing programs. This is suprising since it has 2 gig of RAM and 2.3 Gig processor. Could it be a memory handling problem that's causing OS problems. Probably, the memory didn't handle the OS installation well and the whole system is compromised now.
I am facing a problem on a server which has raid 5 solution (3 disks), the raid controller went down 2 of the disks were off in the Bios.
We added the 3 disks to a different server identical in brand and architecture, the raid controller was able to reconfigure the virtual drive H:.

I tried the following steps but it always failed:
- create a new db with the same name of the lost db;
- put the db in emergency mode;
- stop sql service and replace the mdf file;
- start sql service;
- Run Dbcc checkdb('mydb')
we got the error below:

Hi,since yesterday I have this message everytime I try to open Sql Server"Enterprise manager"; I reinstalled the outfit completely without successany solutionthanksFernand
I'm a SQL Server 2000 user trying to get my head around v2005, in particular the changes from DTS to Integration Services.
I've build a few packages in BIDS and they work fine there, but saving them to either the file system or MSDB locations and running them in Management Studio results in:
Execute Package Utility has encountered a problem and needs to close. We are sorry for the inconvenience.
If I click on the debug button, I get this:
An unhandled exception ('System.IO.FileNotFoundException') occurred in DTExecUI.exe [404].
I've searched the Internet and found a few people who has also experienced this, but not received any answers.
Is there a kind guru out there with some idea of what is going on? Preferably something that doesn't involve re-installing, because the database has already been configured for our Blackberry users and I can't re-do it now.
Good day good people of the forum while carrying out a remote installation i encountered an UNC path error can anyone be of help giving me a comprehensive answer to the problem. am stuck i cant move ahead from that part of the installation
I am unable to debug the first Script Task (not Script Component) since adding a second Script Task in one of my packages. The script completes successfully and goes green but the above error is displayed followed by another which is not mentioned here as I'm sure it's because of the first error. The second script task debugs fine so I know this is not the problem. But if I delete the second Script Task the first task starts debugging.
I have moved the breakpoint around in the first script and the only line that seems to work is the first line within the main method. It's irrelevant what this first line of code does. I know there is nothing wrong with the script tasks themselves as I have an exact copy in another package that debugs just fine.
When I execute the following transaction log backup TSQL in SSMS
BACKUP LOG ProductDB TO DISK = 'D:Database_BackupTransProductDB.trn';
I get this error:
Msg 3201, Level 16, State 1, Line 2 Cannot open backup device 'D:Database_BackupTransProductDB.trn'. Operating system error 21(The device is not ready.). Msg 3013, Level 16, State 1, Line 2 BACKUP LOG is terminating abnormally.
I also have a daily database backup job that puts the backup file under this directory.
D:Database_BackupData
So I changed the path to this and it still fails.
BACKUP LOG ProductDB TO DISK = 'D:Database_BackupDataProductDB.trn';
However if I change it to this path, it works:
BACKUP LOG ProductDB TO DISK = 'MyServerDataProductDB.trn';
This turns out to be the path I used a few weeks back when I backed up the log for the very first time. But I now wanted to store my transaction log backups in D:Database_BackupTrans.
